VIRTUAL HEALTH SURVEY

Before our first virtual health session you are required to fill out the below health history survey. This will allow our first session to start a treatment plan and not waste your time repeating medical information. All medical information is confidential as per the requirements of the MEDICAL CONFIDENTIALITY ACT - Singapore Medical Association. 

“The unauthorised disclosure of information obtained from patients in confidence or in the course of attending to a patient is an offence. Patients are entitled to expect that information about themselves or others which a practitioner learns in his professional capacity, will remain confidential”.
